The simple way to Store your Divi Contact Form submissions in your database.

Divi Form DB

Save form to database

The Divi contact form module is versatile but is often overlooked as it doesn’t natively save form submissions to the WordPress database. Divi Form DB solves that problem, plus more!

Simple CSV exports

Export your data on a per form basis for off line storage or to import it into another application or service.

Just 3 quick clicks and you have your data downloaded.

Copy result to a CPT

Divi Form DB’s most powerful feature is the ability to copy a form submission into any post type (built-in & CPT).

Let your visitors submit blog posts, testimonials & more.

Resend Submissions

Lost the original email? Not a problem, individual submissions can be resent to the site administrator’s email address.

Thank You Email

Have you ever filled in a form and forgot what you submitted? Was my phone number correct? Worry not, as a thank you email can now be instantly sent to the submitter.

BCC Form Submission

Would you like to be on BCC for any form submissions? Now you can with a couple of clicks. Ideal for testing without having to edit multiple forms.

Simple, lightweight & easy to use

Divi Form DB requires zero set up. Once your license key is saved it’ll start working.

New owner & pricing plans

Divi Form DB was acquired in Jan 2024 by myself (Dave Grey), the man behind HeadsUpWP.

From v2.0 it is licensed via Freemius and include new annual prices.

A new license will be required to receive future updates and support. For sites on 1.9.2 or below, please cancel your existing annual license so you can re-subscribe via Freemius.

Using Divi Form DB

Once your license has been activated Divi Form DB will work right out of the box. No complex set up required!

Divi Form DB menu

To view your Divi Contact Form submissions head to “Divi Form to DB” on the admin menu.

From here you can view all submissions, access the export screen and control the admin notification setting.

View Submissions

You can view a form submission by clicking its “View Submission” link in the summary table. It will provide a simple view of your form’s results.

The “Extra information” panel includes the page it was submitted on and who it was submitted by (if logged in).

Export to CSV

From the Export screen you can select the form you would like to export. Click “Preview Submissions” to check it has some results, then click to “Download CSV File” to get your results.

Repurpose results

With Divi Form DB you can also clone your form’s submission to any post type.

Your simple contact form can now be used to submit testimonials, case studies, support tickets, even full blog or news posts.

Select the correct Post Type, map your fields, then click “Clone as draft”. A new entry will be created for you to review and publish as required.

Jan 2025 (V2.2)

Version 2.2 includes:

  • Submission Summary – View submissions by year and month.
  • Bulk Delete – Delete all submissions in one go, or select a custom date range.
  • IP Address logging – Logs submitters IP address (viewable on the individual submission view).

New Emails (V2.1)

Version 2.1 includes 3 new emails:

  • BCC form submission – Includes a copy of any forms to a second email address (ideal for testing, or clients that say they never saw an email).
  • Thank you – Sends a copy of the form submission to the person who submitted it.
  • Resend a submission – Sends a copy of a form submission to the site administrator.

Clone to any post type

You can easily clone your form submission to any built-in or custom post type, for example a testimonial entry.

Select “Custom Field” and provide the matching field’s name as required. Once ready, click “Clone as draft” to create a new record.

Once completed a notification message allows you quickly view, or edit, your entry before publishing it.

Alternatively you can use the “Clone History” panel to see previous activity, handy to avoid creating duplicates.
